vintage Czech artist signed glass, hand blown huge heavy cobalt blue glass plate

Huge (15" diameter) and heavy art glass plate, probably Czech, I think. This is signed Petr, dated 2000. The deep cobalt blue color is lovely, just shines in the sunlight. It's in very good condition, a few light scratches.
